Respected CPIO,\n\nKindly provide following information with certified documents available with you or your department approved by competent authority.\n\n1) My son Ankit Agrawal appeared in JEE Main April 2019 on 8.4.2019 at Ion Ditigal zone, Parthivi College of Engineering and Management, Bhilai 3, Bhilai, Distt. Durg, Chhattisgarh in first shift with Roll Number CG01700140.\nHe was denied Blank Sheet for rough work after issuing six blank sheets by Invigilator Ms. Upasna Chandrakar at lab 12B the C.B.T. Exam hall, he sat idle for about 20 minutes until NTA observer Dr. D.S. Raghuvanshi intervened in the matter and made him available the seventh blank sheet. This is going to make a huge loss to attain higher score as he could not carry out rough work to mark the correct answer. My son is deeply depressed now as he could not mark the answer which he could have done easily if required sufficient blank sheets might have been provided to him.\n2) When the exam. was over he told me the whole matter and contacted Ms. Upasna, Dr. D.S. Raghuvanshi (observer NTA), Dr. Rachna Pandey(Observer NTA), Mr. Tikesh Sinha (TCS Operation Executive) and talked over mobile to Mr. R.S. Pandey, City co-ordinator JEE Main April 2019 over his mobile number 9406096398 at 13:13 Hrs. 8.4.2019 and asked them to show me the Approved Written Order by the Competent Authority in connection with issue of LIMITED NUMBER OF BLANK SHEETS FOR ROUGH WORK.\n3) May please note that as per best of knowledge there is no written instruction in the JEE Main April 2019 Brochure or in the Admit Card JEE Main April 2019 of my son.\n4) Kindly see the attached pdf document for details.\n5) Kindly provide me copy of certified order/ instruction approved by competent authority of NTA or MHRD for issuing restricted numbers of blank sheets for rough work to a JEE Main April 2019 aspirant/ candidate.\n6) Kindly let me know the name, designation, official address, official mail and official contact numbers of the Authority RESPONSIBLE for this HUGE loss to my sons career as he is not going to attain higher score for getting admission in the branch and college of his choice.\n7) Kindly provide me the certified copy of the contract along with attachments carried out between National Testing Agency and Tata Consultancy Services for carrying out JEE Main April 2019 examination through them. \n8) I am ready to pay Cost of Information, kindly provide link for online payment.\n9) Kindly let me know the name, designation address, contact number and mail of the first appellate authority.\n10) Kindly provide information with certified copies withing stipulated time.\n11) Cost of application is paid via internet banking.\nThanks\nMukesh Kumar Agrawal
