My husband, Late Shri Radhey Shyam Gupta (Shri R. S. Gupta), retired as Principal from Government Co-Ed. Senior Secondary School, Vishwas Nagar, Delhi-110 032 on January 31, 1991. His pension Pay Order was issued vide PPO No. 680379100147 dated April 1, 1991 by Pay & Accounts Officer, Central Pension Accounting Office, Government of India, New Delhi. Shri Radhey Shyam Gupta expired on December 24, 2007. When I applied for the revision of pension as per office memorandum No.38/37/08-P&PW (A) of Department of Pension & Pensioners Welfare, Ministry of Personnel, PG & Pensions, I have been asked, by PAO-VIII, G.T.B. Hospital Complex, to submit the Service Book of my husband Late Shri Radhey Shyam Gupta. My son approached the concerned school, however, the school authorities informed that the Service Book is not traceable. As a result, the revision of pension is pending since long. In view of above, it is requested to kindly provide me the following: 1. Attested copies of the Service Book of my husband Late Shri Radhey Shyam Gupta 2. Attested copies of the correspondence & note sheet portions of the relevant file(s) from which the pension case of Late Shri Radhey Shyam Gupta was dealt. 3. Attested copies of the correspondence & note sheet portions of the file in which case for awarding family pension to me (Mrs. Vimlesh Kumari) was dealt with.
