Whether any complaint pursuant to the incident has been filed, and if yes, by whom.\n3. Whether any Enquiry committee has been set up, and if yes, what is the constitution of the committee.\n4. Whether the members of the committee have any conflict of interests, and details thereof. Any present and past association of the DoFA with the committee members may be furnished.\n5. Whether those in a position to exert undue influence on the outcome of the enquiry report have recused themselves from their posts for the duration of the enquiry, and if not, the reasons thereof.\n6. Whether accused, aggrieved and witnesses have been invited by the committee to depose/testify before the committee, and the list of such persons. If not, details may be provided stating the reasons thereof.\n7. The deadline by which the committee is to submit its report, and if completed, the follow-up actions being taken against the offenders.\n8.
